Job Description:
We are seeking a Licensed Practical Nurse (LPN) to join our
skilled nursing and rehabilitation team.
Provide compassionate patient care, supervise nursing assistants,
and ensure adherence to regulations.
Perform administrative tasks like completing medical forms,
documenting observations, and updating electronic health
records.
Administer medications, treatments, and transcribe physician orders
accurately.
Coordinate resident admissions, make rounds with providers, and
monitor patient status.
Ensure care plans are followed and updated to meet residents'
needs.
Must hold an active LPN license and CPR certification; previous
long-term care experience is preferred.
