Job Summary The Mental Health Therapist will support Carilion's hallmarks of service excellence and quality for all job responsibilities. The therapist will provide therapeutic services to patients from young adult through advanced adulthood. Services include provision of initial assessment, individual, family and group therapy. SIGNIFICANT RESPONSIBILITIES: Performs targeted screenings and functional assessments to determine the patient's need for non-medical services within in the primary care setting. Works as a member of the multidisciplinary treatment team. Formulates behavioral health interventions appropriate to primary care Provides individual, family and group therapy. Provides consultation to other members of the medical team. Develops and maintains therapeutic relationships with patients in the individual, family and group setting. Provides education for team members and patients in areas of expertise. Coordinates the patient's treatment with the multidisciplinary treatment team, patient's family, and referral source. Makes appropriate follow-up plans. Records patient data in the medical record to include the assessment, group, family and individual progress notes and follow-up /appointments. Works cooperatively with other members of the treatment team, including ongoing consultation for complex and difficult patients. The Mental Health Therapist refers patients out to specialty mental health care when appropriate. Observes and documents data reflective of the patient's status, determines the relevant information to identify each patient's requirements relative to his or her specific needs, and provides the care needed. MINIMUM REQUIREMENTS: Education: LCSW (Licensed Clinical Social Worker) or PHD (Licensed Clinical Psychologist) Experience: One to three years of related experience required.
