If so, consider joining our dynamic, educated team of Mental Health Professionals who work on Interdisciplinary Teams in Carilion's inpatient psychiatry units, in Roanoke, Va! Interested candidates should be prepared to utilize mental health therapies such as SFBT, coordinate with community partners, facilitate (or, co-facilitate) group therapy sessions, help patients plan for hospital discharges, and, advocate for patients' needs during their admission to our hospital. We are looking for a dedicated professional to support our FT weekday staff so as to improve the quality of our patient care! Flexible Weekday hours between 8AM - 5PM. Holiday coverage shared between our department. The clinical social worker provides psychosocial assessments, interviews, and/or treatments and discharge planning to patients and families to assist them in coping with hospitalization, illness, diagnosis, and/or life situation. The social worker works collaboratively with other members of the health care team, community resources, regulatory agencies and third party payers in order to facilitate the transition of the patient through the organization while maintaining the highest quality of services possible. The social worker will adhere to the National Association of Social Workers Code of Ethics. The clinical social worker utilizes critical thinking skills, advanced problem solving, therapeutic interventions and advanced care planning in the day to day functions. Duties include, but are not limited to the following: -Provide psychosocial assessments of patients and families. Formulating a comprehensive care transition plan coordinated with the interdisciplinary/ transdisciplinary team. -Meets documentation standards for quality, timeliness and confidentiality of medical records in accordance with organizational and departmental defined policies and procedures. - -Remain current with social work and health care developments and seeks to increase further enhancement of job related knowledge. -Facilitates and interdisciplinary approach to patient care. -Provide therapeutic interventions aimed at adaptation throughout the continuum of care. - -Participates in system and departmental performance improvement initiatives. What We Require: Education: Master's Degree from an accredited school of Social Work. Experience: Minimum of two years social work experience in health care setting preferred. Other Minimum Qualifications: Effective interpersonal, communication, and team working skills required. Must successfully complete orientation and competency validation for position. Demonstrates advanced knowledge of social work theory and practice: proven expertise as a clinical social worker; crisis intervention skills; critical thinking and creative problem solving. Satisfactory completion of orientation; positive interpersonal skills; oral and written communication skills; analytical skills; cultural competence; integrity; team player; courteous; ability/willingness to work as an integral member of a multi skilled team.
