How You'll Help Transform Healthcare: Home Health- Full Time Provides clerical and scheduling services necessary to the operation of the department. -

• Performs scheduling functions critical to the efficient operation of the home health and hospice agencies including generating working calendars for all team members, updating working calendars as necessary to reflect call in's or other scheduled/unscheduled absences.
• Collaborates with the central intake staff to initiate a schedule on all new admissions. -Assigns team visits on a daily basis. -Assigns case managers to new admits. -Accounts for all visits to schedule, adding missed visits as needed. -Reschedules missed visits as appropriate.
• Collaborates with agency managers and clinical team leaders and coordinates schedules to reflect inservices, orientation, PTO, etc. -Schedules staff such that productivity targets are calculated and consistently achieved.
• Confirms accuracy and completion of scheduled visits. -Reviews weekly all open/assigned visits from the previous week and verifies that they were completed or documented. -Follows up appropriately on any open visits and informs clinical team leaders/agency managers of trends or patterns in missed productivity or incomplete documentation.
• Tracks recerts weekly to ensure that recert visits are assigned and completed. -Tracks hospitalizations and alerts case managers to do OASIS transfer documents. -Follows-up to ensure OASIS documents are completed.
• Demonstrates appropriate phone etiquette when dealing with customers and staff. -
• Ensures appropriate hand-off of communication from Central Intake to scheduling to staff.
• Implements appropriate tracking systems to ensure that all referrals are appropriately scheduled pre and post admission. What We Require: Education: HS diploma or equivalent education required. Completion of an accredited medical secretarial or business program preferred. Computer and word processing knowledge and experience required. Experience: One to three years of related experience required. Home Health experience preferred. Keyboarding skills essential and will be verified/validated through appropriate testing. Other Minimum Qualifications: Effective interpersonal, communication and teamwork skills required. Must successfully complete orientation and competency validation for position. Above average organizational skills with attention to detail work. Strong skills in reading, writing, spelling, grammar, punctuation, and mathematical calculations.
