Job Description Job Description Hours: full-time (37.5 hours per
week), Day Shift Location: Lynchburg VA, 24501 Compensation
dependent on experience Position Summary The LPN - PACT is
responsible for the overall medical care of assigned individuals,
including the delivery service of basic nursing assessments;
delivery of client/family education and diagnostic procedures;
consultation with physicians; and carrying out of doctor's orders.
75% of nursing care is provided in the community setting. Required
Education and Experience Education Requirements The successful
completion of a course of study that allows for licensure as a
Licensed Practical Nurse in the state of VA is required. Types of
Experience One year of experience providing nursing services as an
LPN is required or an equivalent combination of education and
experience that provides the required knowledge, skills, and
abilities. Related Knowledge, Skills and Abilities Knowledge of
community mental health and public health in order to provide
advocacy and enhance an individual’s treatment and access to
community resources. Knowledge of nursing best practice. Knowledge
of the principles, procedures, techniques, trends, and literature
related to mental health, physical health, intellectual
disabilities and substance abuse. Knowledge of psychiatric/medical
medications/treatments. Ability to safely administer medications.
Ability to analyze situations accurately and develop appropriate
responses or techniques. Ability to communicate effectively in
order to facilitate treatment and relay information to others as
part of an interdisciplinary team. Ability to perform medical
interviews in order to obtain information and to formulate
appropriate treatment plans and referrals. Ability to produce,
read, and relay medical lab results. Ability to develop and
maintain therapeutic relationships. Ability to promote positive,
collaborative, and professional working relations among co-workers
and representatives in the community. Ability to work in and
contribute to a collaborative team environment. Ability to respond
effectively to changes in the work environment in a positive and
professional manner. Ability to identify and measure outcomes.
Willing and available to work modified schedules (weekends,
